Referral
Patients are referred by their treating physician, usually once other treatment options have been exhausted. Patients who feel unsupported and discouraged about losses from their injury, who are angry at their circumstances, are depressed due to realizing they may not get "back to normal" are considered candidates. Those who are willing to work to get their lives back in order are considered ideal candidates.

FAX REFERRAL: If you prefer, print the Referral Form page or download the PDF form, fill it out and fax it to us at (510) 985-1191.
We will contact the insurance company claims representative for an authorization and will communicate directly with physicians, claims administrators, case managers, and attorneys regarding progress toward this end. We will obtain needed medical records and contact all parties for scheduling when authorization is obtained.
